No specific medication, therapy, or outcome is guaranteed
We reserve the right to decline or discontinue treatment based on medical judgment, safety, or compliance concerns
If telemedicine services are offered:
You acknowledge inherent limitations of remote care
You consent to telehealth services where legally permitted
You are responsible for providing accurate information and a suitable environment for telehealth visits
You agree to:
Provide accurate, complete, and truthful medical information
Follow agreed-upon treatment plans
Ask questions when clarification is needed
Use services respectfully and lawfully
You agree not to:
Misuse or attempt to disrupt the website
