Foundations Tier Quick facts

 

  • FDA-approved medications only (including Wegovy®, Zepbound®, phentermine, ect.)

Medications used in the Foundations Tier

 

The Foundations tier uses FDA-approved, brand-name weight-loss medications, prescribed on-label and according to established medical criteria.

Adults with:

  • BMI ≥ 30 kg/m² (obesity), or

  • BMI ≥ 27 kg/m² (overweight) with at least one weight-related comorbid condition (commonly listed examples include hypertension, dyslipidemia, type 2 diabetes, obstructive sleep apnea, etc.).

Depending on eligibility and medical history, medications may include:

  • Wegovy® (semaglutide)

  • Zepbound® (tirzepatide)

  • Phentermine (when appropriate and medically indicated)

  • Other FDA-approved anti-obesity medications, selected case-by-case

Medication selection is individualized and based on:

  • On-label prescribing criteria

  • Medical history and current conditions

  • Blood pressure and heart rate

  • Prior response or intolerance to medications

  • Side-effect risk and contraindications

Not every patient qualifies for every medication, and no medication is guaranteed.

Visit schedule and follow-up expectations

 The Foundations tier is designed to be efficient, structured, and medically appropriate, with a predictable follow-up schedule.

During medication initiation and dose adjustment

  • Patients are seen monthly

  • This applies when starting or adjusting:

    • Wegovy®

    • Zepbound®

    • Phentermine or other approved medications

  • Monthly visits focus on:

    • Medication response and tolerability

    • Side-effect monitoring

    • Vital signs and safety review

    • Dose adjustments when appropriate

Monthly visits continue only until a stable, effective dose is reached.

Once on a stable dose

  • Follow-up transitions to every 3 months

  • Maintenance visits focus on:

    • Ongoing safety and medical appropriateness

    • Weight and metabolic trend review

    • Continued need for Wegovy®, Zepbound®, phentermine, or other therapy

    • Reinforcement of nutrition and activity basics

Additional visits are not routine and are scheduled only if medically necessary.

How the process works 

Step 1 — Schedule your evaluation

Initial in-person visit and labs see labs page

Step 2 — Monthly visits during titration

You are seen once per month while starting or adjusting Wegovy®, Zepbound®, phentermine, or other approved medications.

Step 3 — Transition to maintenance

After reaching a stable dose, visits move to once every 3 months.

Step 4 — Ongoing care

Continued prescribing and monitoring as long as the medication remains safe, appropriate, and effective.

How often will I see the doctor?

  • Monthly visits while starting or adjusting medication

  • Every 3 months once you are stable

Why aren’t visits more frequent once I’m stable?
For patients on a stable dose of Wegovy®, Zepbound®, or phentermine, more frequent visits do not improve outcomes and are not medically necessary.

Can I request extra visits?
Additional visits may be scheduled for side effects, safety concerns, or medical issues. Routine extra visits are not part of the Foundations tier.
