What is BMI and how is it used?

BMI (body mass index) gives clinicians a quick way to screen for weight category by comparing a person’s weight to their height (BMI = weight in kg ÷ height in meters²). Clinicians calculate BMI and then place it into standard adult categories that generally track with rising health risk: underweight (<18.5), healthy weight (18.5–24.9), overweight (25–29.9), and obesity (≥30), with obesity further divided into Class 1 (30–34.9), Class 2 (35–39.9), and Class 3 (≥40). Even though BMI does not directly measure body fat percentage or fat distribution, clinicians still rely on it because it standardizes documentation, supports consistent risk stratification, and prompts the next steps in evaluation.

In practice, clinicians use BMI as a starting point to qualify patients for different levels of weight-loss treatment, and then they layer in comorbidities, prior attempts, and overall risk. For example, when clinicians consider FDA-approved chronic weight-management medications, they typically use thresholds such as BMI ≥30, or BMI ≥27 when the patient also has at least one weight-related condition (for example hypertension, dyslipidemia, type 2 diabetes, obstructive sleep apnea, or cardiovascular disease), alongside nutrition and activity interventions. Likewise, when clinicians evaluate patients for metabolic/bariatric surgery, they often apply BMI-based criteria (commonly BMI ≥40, or BMI ≥35 with significant comorbidities); however, newer professional guidance also supports surgery for BMI ≥35 regardless of comorbidities and allows consideration for BMI 30–34.9 in selected higher-risk patients when non-surgical therapy has not produced durable results.
